Once you have scheduled an appointment as a new patient, by calling 719-358-5170, please fill out the appropriate form below.

These forms are intended to be filled out online and be either e-mailed back to us or printed and brought with you to your appointment. If you are unable to complete them online and would prefer to complete them in our office, please let us know and plan to arrive a minimum of 30 minutes prior to your appointment to do so.

If your forms are not completed at the start time of your appointment, we may need to reschedule you to another time.
